Getting started is easy. Here's everything you need to know before your first visit.
Booking your first visit
- Call us at (631) 727-0770. We book all appointments by phone, Monday through Thursday, 8 AM to 5 PM.
- Get your new patient forms. Just ask when you call, and we'll mail them to you so you can fill them out ahead of time.
- Bring your insurance card. We accept many PPO plans and are happy to check your coverage.
- Tell us what's on your mind. Your first visit starts with listening. Nervous? Let us know — we'll take it slow.
